ABSTRACT

This paper aimed to study the effect of Dalbergia cochinchinensis heartwood on plasma endogenous metabolites in rats with ligation of the left anterior descending coronary artery, and to analyze the mechanism of D. cochinchinensis heartwood in improving acute myocardial ischemic injury. The stability and consistency of the components in the D. cochinchinensis heartwood were verified by the establishment of fingerprint, and 30 male SD rats were randomly divided into a sham group, a model group, and a D. cochinchinensis heartwood(6 g·kg~(-1)) group, with 10 rats in each group. The sham group only opened the chest without ligation, while the other groups established the model of ligation. Ten days after administration, the hearts were taken for hematoxylin-eosin(HE) staining, and the content of heart injury indexes in the plasma creatine kinase isoenzyme(CK-MB) and lactate dehydrogenase(LDH), energy metabolism-related index glucose(Glu) content, and vascular endothelial function index nitric oxide(NO) was determined. The endogenous metabolites were detected by ultra-high-performance liquid chromatography-time-of-flight-mass spectrometry(UPLC-Q-TOF-MS). The results showed that the D. cochinchinensis heartwood reduced the content of CK-MB and LDH in the plasma of rats to relieve myocardial injury, reduced the content of Glu in the plasma, improved myocardial energy metabolism, increased the content of NO, cured the vascular endothelial injury, and promoted vasodilation. D. cochinchinensis heartwood improved the increase of intercellular space, myocardial inflammatory cell infiltration, and myofilament rupture caused by ligation of the left anterior descending coronary artery. The metabolomic study showed that the content of 26 metabolites in the plasma of rats in the model group increased significantly, while the content of 27 metabolites decreased significantly. Twenty metabolites were significantly adjusted after the administration of D. cochinchinensis heartwood. D. cochinchinensis heartwood can significantly adjust the metabolic abnormality in rats with ligation of the left anterior descending coronary artery, and its mechanism may be related to the regulation of cardiac energy metabolism, NO production, and inflammation. The results provide a corresponding basis for further explaining the effect of D. cochinchinensis on the acute myocardial injury.

ABSTRACT

Objective The purpose of this study was to compare two types of myocardial infarction ( MI) models in Beagle dogs. Methods 30 dogs were divided randomly into three groups ( n =10 ) . ① The sham?operated group underwent pericardiotomy but without coronary artery ligation. The other two model groups were made under video?assisted thoracoscopy ( VATS) , where the left anterior descending coronary artery was closed by titanium clips:②The direct vision group:the minimally invasive closure of the artery was performed under direct vision. In this group, the thoracoscopic operation was performed through a 3?0 cm small incision opened at the margin of the left third rib. ③ The thoracoscopic group:the video?assisted thoracoscope was inserted into the chest through a 1?0 cm exploratory hole in the midline of the third rib, and the surgical instruments were inserted through two 0?5 cm operating holes at the para?sternum line of the third rib and midclavicular line of the fourth rib. Electrocardiogram ( ECG) was recorded and the levels of serum creatine kinase?MB ( CK?MB) and troponin I ( cTnI) were measured after modeling. The heart tissue samples were examined by histology using HE staining. The success rate of model establishment, durations from skin incision to chest closing and the wound healing were recorded. Results Compared with the sham?operated group, changes of myocardial infarction were observed in the two model groups ( ECG S?T segment elevation, increased serum CK?MB and cTnI levels, myocardial ischemia and fibrosis, and reduced amount of cardiomyocytes ) . The survival rate was 90% in both of the two model groups. The operating time was shorter in the minimally invasive surgery under direct vision group, and the wound healing time was shorter in the thoracoscopic group. Conclusions The myocardial infarction models generated by minimally invasive surgery have less trauma and low mortality in the dogs. This model is suitable for investigation of pathophysiological mechanism associated with myocardial infarction.

ABSTRACT

Objective To explore the significance of treadmill exercise test (TET) in identifying left main coronary artery(LMD) obstruction from proximal left anterior descending(LAD) obstruction.Methods Patients with positive in the TET of the Central Hospital of Dalian from Jan.2008 to Dec.2013 were enrolled in this study and they were divided into LMD group with 21 cases and LAD group with 35 cases according to the results of coronary angiography.Retrospectively study was conducted to analyze the exercise time,the start time of ST segment change,the lead of ST segment between groups.Results Lead aVR ST segment elevation(≥0.10 mV) was 85.7% (18/21) in the LMD group and 62.9% (22/35) in the LAD group.Lead aVR ST segment elevation in the LM group was ((0.16 ± 0.07) mV,higher than in the LAD group ((0.11 ± 0.07) mV ; t =2.614,P < 0.05).Accompany ST segment depression of leads and cumulative ST segment depression were (5.7 ±1.7) pcs and (1.13 ±0.42) mV in LMD group,significantly higher than those of LAD group((4.6 ± ± 1.8) pcs and (0.87 ± 0.32) mV ; t =2.314,2.634,P < 0.05).The occurrence rate of ST segment depression (≥0.10 mV) in leads V2 and V6 were 33.3% (7/21) and 90.5% (19/21) in LM group,significantly higher than that in LAD group (11.4% (4/35) and 62.9 % (22/35) ; x2 =3.990,5.105 ;P < 0..05).The start time of ST segment deviation of LM group was significantly earlier than that of LAD group ((199.8 ± 70.8) s vs.(273.7 ± 127.3) s;t =-2.787,P < 0.01).The differences were not statistically significant in terms of the exercise time between groups ((315.0 ± 120.0) s vs.(376.1 ± 110.0) s;t =-1.948;P > 0.05),the rate for achieving target heart rate(33.3% (7/21) vs.48.6% (17/35) ; x2 =1.244 ; P > 0.05) and the Duke score ((-11.8 ±7.2) vs.(-8.4 ±6.8) ;t =-1:772;P>0.05).Conclusion Compare with patients with the LAD disease,ST segment elevation of lead aVR is significantly higher induced by treadmill exercise test in patients with left main disease.Meanwhile,the lead accompanied with ST segment depression is significantly higher,and the start time of ST segment deviation is significantly earlier.

ABSTRACT

Single right coronary artery, so defined because all three coronary arteries arise from the single right aortic sinus of Valsalva, is an extremely rare entity occurring in less than 0.03% of the population. This case identifies a single coronary artery with the left anterior descending coronary artery distally connected to the left pulmonary artery with development of pulmonary hypertension. To the author's knowledge, this has never been documented in the English literature.


La arteria coronaria derecha única, definida así porque las tres arterias coronarias salen de un solo seno aórtico derecho de Valsalva, es una entidad extremadamente rara, que ocurre en menos de 0.03% de la población. Este caso identifica una arteria coronaria única con la arteria coronaria descendente anterior izquierda, conectada de forma distal a la arteria pulmonar izquierda con desarrollo de hipertensión pulmonar. Hasta donde el autor conoce, un caso como éste no ha sido nunca reportado en la literatura existente en inglés.

ABSTRACT

Objective To study the effect of left ventricular function in the left anterior descending coronary artery stent implantation.Methods randomly 100 cases of left anterior descending coronary artery stenosis in patients with coronary heart disease branch were divided into the observation group and the control group (each group 50 cases),the control group used mainly drug treatment,the observation group used a implanted stent in the treatment of percutaneous left anterior descending coronary artery.the left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD,left ventricular ejection fraction(LVEF) and serum N terminal pro-brain natriuretic peptide(NT-proBNP) levels were observed between two groups.Results after treatment,6 months,two groups of LVEDD,LVESD and NT-proBNP compared with those before treatment were significantly lower(t =2.75,3.14,3.54,all P <0.05),while LVEF increased significantly (t =12.45,P < 0.05);and compared with the control group,LVEDD,LVESD of the observation group decreased significantly,LVEF increased significantly(t =2.27,3.75,2.21,all P < 0.05).Conclusion The left anterior descending coronary artery stent implantation in treatment can effectively improve left ventricular function in patients with coronary heart disease.

ABSTRACT

La anatomía normal de las arterias coronarias incluye una serie de variantes respecto a su origen, distribución y recorrido, que hacen que no existan 2 pacientes con coronarias exactamente iguales. La arteria descendente anterior puede tener variantes anatómicas que no afectan el aporte normal de sangre al territorio miocárdico correspondiente. En nuestros 25 años de trabajo y más de 11,000 coronariografías realizadas, sólo habíamos encontrado un paciente con un doble sistema, tipo IV, de la arteria descendente anterior. En este artículo mostramos el caso de una paciente con la anomalía coronaria descrita que presentaba, además, una estenosis severa de la arteria circunfleja que fue tratada exitosamente por vía percutánea. También, se propone una actualización de la clasificación de Spindola-Franco, donde se mantienen los 4 tipos originales y se añaden 7 variantes anatómicas o subtipos.


The normal anatomy of coronary arteries includes a kind of variants with regard to its origin, distribution and route, which makes that 2 patients do not have coronary arteries exactly alike. Left anterior descending artery could have anatomical variations that do not affect the normal supply of blood to the corresponding myocardial territory. In our 25 years of experience, and more than 11,000 coronary angiographies, we have found only one patient with a type IV Classification; dual left anterior descending coronary artery. In this article we show the case of a patient with the described coronary anomaly, presenting a severe stenosis of the circumflex artery which was successfully treated percutaneously. Besides, a proposal for updating the classification of Spindola-Franco is made, where the 4 types previously described were kept invariable, and 7 anatomical variants or subtypes were added.

ABSTRACT

Objective To delineate and measure the left anterior descending coronary artery (LAD) on CT angiography (CTA) and evaluate the dose delivered to LAD by different radiotherapy techniques for left-sided breast cancer.Methods Twenty-nine left-side breast cancer women with mean age of 54.71 years (range:30-80 years) were included.CTA was performed, and LAD was contoured and the distances were measured between LAD and chest wall (posteroanterior diameter,PD), between LAD and internal mammary artery (horizontal diameter,HD), between LAD and interventricular groove (oblique diameter,OD) at the level of T7-T8,T8-T9,T9-T10 and at level of nipple and lower boundary of the breast.The dose delivered to LAD was calculated on three-dimensional plans for two patients with mastectomy whose chest wall and internal mammary chain (IMC) were irradiated and one patient with breast-conserving surgery who received whole breast irradiation.Results The LAD arose at the level of the third rib in 40% of patients and at the fourth rib in 60% of patients.The mean length of LAD was 7.49±0.58 cm.At the level of T7-T8,T8-T9,T9-T10,the mean PD were 2.99±1.11 cm, 1.26±0.65 cm,0.68±0.39 cm, the mean HD were 2.27±0.84 cm, 2.81±0.65 cm, 3.37±1.21 cm, and the mean OD were 0.47±0.25 cm,0.38±0.21 cm,0.42±0.19 cm respectively.At the level of the breast nipple and the lower boundary of the breast, the mean PD were 2.94±1.06 cm, 0.79±0.46 cm, the mean HD were 2.45±0.89 cm, 3.31±1.22 cm,and the mean OD were 0.56±0.30 cm,0.57±0.24 cm respectively.The mean dose to the LAD was 5 Gy and 14 Gy for patients with mastectomy whose IMC was irradiated with 9 MeV electron and whose IMC was irradiated with 6 MV photon tangential beams.The mean dose to the LAD was 26 Gy for patients with breast conserving surgery.Conclusions To contour the LAD, the interventricular groove could be the reference point.Tangential technique can be giving a higher dose of LAD when compared with other radiation techniques

ABSTRACT

Objective: To assess the regional systolic and diastolic function of left ventricle before and after coronary stent implantation by pulsed wave Doppler tissue imaging (PW-DTI). Methods: Twenty-six patients with coronary artery disease ( CAD) involving only left anterior descending coronary artery ( LAD) were examined within 1-3 days before, 7 days and 30 days after coronary stenting with PW-DTI. Peak systolic , peak early diastolic, and peak late diastolic motion velocities ( Sm, Em, and Am respectively) were measured at 6 different sites on the basal and medial segments corresponding to the anteroseptal, postero-septal, lateral, anterior, inferior, and posterior walls of the left ventricle. Results:In corresponding segments depending on LAD blood flow supply, Sm was increased significantly 7 days after the stenting proce- dure. Em was also increased 7 days after the procedure in most of the corresponding segments. There were little changes in other segments that were independent on LAD blood flow supply. Conclusion: These findings suggest that there is an early improvement in the regional systolic and diastolic function of the left ventricle after the coronary stenting, and that PW-DTI techniques could evaluate quantitatively the regional ventricular performance in patients receiving percutaneous coronary intervention.
